Testing Instructions For the Saliva Cups:

 

In order to ensure accurate test results please do not eat any food 30 minutes prior to giving your sample. Best practice is to drink water prior to giving sample:

 

Step 1: Remove the vial out of the specimen bag.

Step 2: Place your lips to the edge of the tube and drop 3 drops of saliva into the tube 

Step 3: Close the vial and write your full name and DOB (date of birth) on the vial itself. 

Step 4: Place the vial in the specimen bag and seal the bag. 

Step 5. Fill out the form, make sure to fill in all details and answer the questions.

Step 6: Place the form in the sleeve of the specimen bag. 

Step 7: Include a copy of the front and back of your insurance card in the sleeve of the specimen bag. 

 

Testing Instructions For the Nasal Swabs:

 

In order to ensure accurate test results please do not eat any food 30 minutes prior to giving your sample. Best practice is to drink water prior to giving sample:

 

Step 1: Remove the vial out of the specimen bag.

Step 2: Open nasal swab

Step 3: Remove the nasal swab from the wrapper by pulling the two ends of the wrapper apart (like you would to open a band-aid). Be careful to only touch the handle, not the tip.  

Step 4: Gently insert the entire soft tip of the swab into one nostril until you feel a bit of resistance and rub it in a circle around your nostril 4 times.

Place the vial in the specimen bag and seal the bag. 

Step 5: Next, gently insert the same swab into the other nostril and rub it around the same way. 

Step 6: Lower the swab, tip first, into the provided tube. Once the tip is at the bottom, break the swab handle at the top of the tube by bending back and forth. Screw the red cap on tightly

Step 7: Fill out the form, make sure to fill in all details and answer the questions.

Step 8: Place the form in the sleeve of the specimen bag. 

Step 9: Include a copy of the front and back of your insurance card in the sleeve of the specimen bag.
